Before submitting an application, please read through the requirements below:

☑   I understand that Stars and Stripes Honor Flight, Inc. flies veterans out of Milwaukee, WI. 

☑   I know a veteran who has already applied and I intend to be their guardian

☑   I am between 18-64 years of age (65-69 may be considered with a medical review by our doctors)

☑   I am at least 15 years younger and one generation removed from my veteran

☑   I am not the spouse or significant other of my veteran

☑   I am intellectually, physically and emotionally capable of caring for my veteran

☑   I understand that there is a $500 guardian donation to participate as a guardian
