☑   I would like to fly out of MILWAUKEE, WI. (If not, please visit www.honorflight.org for other hubs)

☑  I served active duty (beyond boot camp), honorably, in ANY BRANCH, ANYWHERE, IN ANY CAPACITY between December 1941- June 1975

☑   I understand I will be placed on a fly list and will be contacted approximately 6 weeks before I am assigned to a flight. I also understand that Stars and Stripes Honor Flight, Inc., reserves the right, at its sole discretion, to refuse, deny or reject any applicant or application for any reason at any time from participating in our mission
